ABSTRACT

Paget's disease, described by Sir James Paget in 1874, is classified as mammary and extramammary. The mammary type is rare and often associated with intraductal cancer (93-100% of cases). It is more prevalent in postmenopausal women and it appears as an eczematoid, erythematous, moist or crusted lesion, with or without fine scaling, infiltration and inversion of the nipple. It must be distinguished from erosive adenomatosis of the nipple, cutaneous extension of breast carcinoma, psoriasis, atopic dermatitis, contact dermatitis, chronic eczema, lactiferous ducts ectasia, Bowen's disease, basal cell carcinoma, melanoma and intraductal papilloma. Diagnosis is histological and prognosis and treatment depend on the type of underlying breast cancer. Extramammary Paget's disease is considered an adenocarcinoma originating from the skin or skin appendages in areas with apocrine glands. The primary location is the vulvar area, followed by the perianal region, scrotum, penis and axillae. It starts as an erythematous plaque of indolent growth, with well-defined edges, fine scaling, excoriations, exulcerations and lichenification. In most cases it is not associated with cancer, although there are publications linking it to tumors of the vulva, vagina, cervix and corpus uteri, bladder, ovary, gallbladder, liver, breast, colon and rectum. Differential diagnoses are candidiasis, psoriasis and chronic lichen simplex. Histopathology confirms the diagnosis. Before treatment begins, associated malignancies should be investigated. Surgical excision and micrographic surgery are the best treatment options, although recurrences are frequent.

ABSTRACT

O controle da hanseníase depende de um tratamento com a dose completa em até 18 meses, o abandono implica na transmissão e em quadros de incapacidade física. OBJETIVO: O objetivo da pesquisa foi identificaro perfil clínico-epidemiológico dos pacientes que abandonaram o tratamento de hanseníase em Teresina-PI.MATERIAL E MÉTODOS: Trata-se de um estudo de natureza descritiva, transversal, com abordagem quantitativa. Os dados referentes a esses indivíduos nas coortes de 2011 a 2014 foram obtidos por meio do Sistema de Informações de Agravos de Notificação.RESULTADOS: Do total de 1770 casos de hanseníase notificados nesse período, 34 casos foram considerados abandono de tratamento. Verificou-se que 54,9%eram homens, 79,5% estavam na faixa etária de 15 a59 anos, 68% eram pardos e 59% tinham até 3 anos de escolaridade . A maioria dos casos de abandono era pacientes multibacilares (61,8%) e 32% apresentava aforma dimorfa. A maior parte dos diagnósticos deu-se por demanda espontânea (55,8%). No momento do diagnóstico, 29,4% dos pacientes possuía algum graude incapacidade física. Pacientes multibacilares tiveram uma média de 3,6 contatos, sendo que cerca da metade foram examinados. Com relação à suspensão da poliquimioterapia, observa-se que paucibacilares tomaram 1,84 doses, enquanto multibacilares, 6,94 doses. CONCLUSÃO: Os multibacilares apresentaram menor adesão ao tratamento, evidenciando maior risco de transmissão. Fazem-se necessárias campanhas de detecção da doença, para evitar sua evolução e as consequentes incapacidades físicas, e ainda reduzir possibilidade de transmissão entre contatos.


Leprosy control depends on a treatment based on full dose within 18 months, the dropping out of the treatment implies in transmission and development of disability. OBJECTIVE: The objective of the study was to identify the clinical and epidemiological profile of patients who dropped out the leprosy treatment in Teresina, PI. MATERIAL AND METHODS: It is a descriptive, transversal study with a quantitative approach. Data relating to individuals in the cohorts from 2011 to 2014 were obtained from the Notifiable Diseases Information System. RESULTS: From the total1770 leprosy cases reported in the period, 34 cases were considered treatment dropouts, 54.9% of them were men, 79.5% aged between 15 and 59 years, 68% were from brown and 59% had up to 3 years of schooling. Most of the abandonment cases were multibacillary patients (61.8%) and 32% had borderlineleprosy. The majority of the diagnosis was made byspontaneous demand (55.8%). At diagnosis, 29.4% ofpatients had some degree of disability. Multibacillary patients had an average of 3.6 contacts, and about half were examined. Regarding the suspension of multidrug therapy, it was observed that paucibacillary took 1.84 doses, while multibacillary, 6.94 doses. CONCLUSION: Multibacillary patients showed lower adherence to treatment, presenting higher risk of transmission. Campaigns for active search of new cases are necessary to prevent disease evolution and the resulting physical disabilities , while reducing the possibility of transmission between contacts.

ABSTRACT

Objetivos: Avaliar a associação do diagnóstico de câncer de pele com fatores de risco conhecidos através da análise de dados da Campanha Nacional de Prevenção ao Câncer de Pele realizada pela Sociedade Brasileira de Dermatologia no estado do Piauí, Brasil, nos últimos anos. Métodos: Estudo transversal analítico com características descritivo-quantitativas utilizando os dados obtidos a partir da referida campanha no estado do Piauí, nos anos de 2009 e 2010. Realizou-se análise descritiva dos dados coletados, além de regressão logística multivariada, usando como variável dependente o diagnóstico de câncer de pele. Resultados: Em 2009 e 2010, esta campanha foi responsável pela realização de 1141 atendimentos, diagnosticando 122 (10,7% dos atendimentos) casos de câncer de pele: 108 carcinomas basocelulares (CBC), 10 espinocelulares (CEC) e 4 melanomas. Dos examinados, 35,4% eram homens, 73,1% relataram proteção solar inadequada, 16,4% apresentavam história familiar de câncer de pele e 7,2% história pregressa. Indivíduos com história pregressa de câncer de pele apresentaram 5,24 vezes mais chances de ter um novo diagnóstico de câncer, ao passo que aqueles com pele não-negra apresentaram 4,91 vezes mais chances de diagnóstico. Conclusão: História pessoal e familiar de neoplasia epitelial, cor não-negra e sexo masculino estiveram associados com maiores chances de desenvolvimento de câncer de pele. Além disso, a exposição solar sem proteção continua sendo rotineira.


Objectives: To evaluate the correlation between the diagnoses of skin cancer and known risk factors through analysis of data from the National Skin Cancer Prevention Campaign held by Brazilian Society of Dermatology in the state of Piauí, Brazil, in recent years. Methods: Cross-sectional descriptive and analytical report using quantitative data obtained from a prevention campaign in the state of Piauí, in 2009 and 2010. Collected data was submitted to a descriptive analysis, and multivariate logistic regression, using as dependent variable the skin cancer diagnosis. Results: In 2009 and 2010, this campaign was responsible for 1141 consultations, diagnosing 122 (10.7%) cases of skin cancer: 108 basal cell carcinomas (BCC), 10 squamous cell (SCC) and four melanomas. Of those examined, 35.4% were male, 73.1% reported inadequate sun protection, 16.4% had a family history of skin cancer and 7.2% had personal history. Those with history of skin cancer were 5.24 times more likely to have a new diagnosis of cancer, while those presenting non-black skin were 4.91 times more likely to diagnosis. Conclusion: Personal or family history of epithelial neoplasia, non-colored black skin and the male gender were associated to higher chances of developing skin cancer. In addition, unprotected sun exposure remains routine.

ABSTRACT

O carcinoma basocelular é o tumor maligno mais frequente do ser humano, sendo a face, a sua localização mais comum. A excisão de lesões iguais ou maiores do que 2 cm no lábio superior, requer reconstrução complexa e difícil. Relata-se o caso de uma paciente do sexo feminino, de 74 anos, portadora de carcinoma basocelular nodular, de ± 2 cm de diâmetro, localizado na metade esquerda do lábio superior e invadindo parte do vermelhão. A lesão foi excisada e a reconstrução feita com retalho de transposição naso-labial, com base inferior. O histopatológico mostrou ressecção completa e o resultado estético após 1 ano mostrou-se satisfatório.

ABSTRACT

O objetivo do trabalho foi avaliar a prevalência das dermatoses dos pacientes atendidos no ambulatório de Dermatologia da Santa Casa de São Paulo, no período de junho a outubro de 2005, e comparar os resultados com levantamento semelhante realizado no mesmo serviço, no ano de 1977. Foram analisados os prontuários eletrônicos de 3.011 pacientes atendidos em primeira consulta.


The objective was to evaluate the prevalence of skin diseases in patients seen at the Dermatology outpatients' clinic of the Santa Casa de São Paulo during the period from June to October 2005, comparing the results with a similar survey conducted at the same center in 1977. During the period of study, the electronic medical charts of 3011 patients seen in a first visits were analyzed.

ABSTRACT

Relatamos o caso de um paciente apresentando carcinoma basocelular nodular ulcerado de 3 cm de diâmetro na região frontal, à esquerda, atingindo a metade medial do supercílio. A lesão foi excisada e a reconstrução, realizada com retalhos de avanço em H. A região esquerda incluiu o restante do supercílio, sendo maior do que a contralateral. Assim, mantiveram-se a harmonia e a simetria das regiões superciliares e glabelar. As cicatrizes fi caram pouco perceptíveis por estarem posicionadas nas linhas de expressão. O paciente permanece sem lesões após dois anos de acompanhamento.


We report the case of a patient presenting a 3 cm diameter ulcerated nodular basal cell carcinoma in the frontal left region, reaching the medial half of the eyebrow. The lesion was excised and the reconstruction was carried out with advancement fl aps in H and the left included the rest of the eyebrow, being larger than the contralateral. Thus, the harmony and symmetry of the superciliary and glabellar regions were remained. The scars were visible just by being positioned in wrinkles rows. The patient remains without injuries after two years of monitoring.
